Collecting audit information has always been an essential element of the health visiting service that we provide for our clients. The review of care is paramount for the improvements of standards and planning future care and services. However, it can be a lengthy process and unfortunately can be daunting for many teams. Therefore this month I have included a copy of a recently formatted birth book page. This can be down loaded and placed in a file, which will effectively become the birth book.

It includes information that reflects recent changes in the child health programme guided by H4ALL.
